Dear NTPS Families, 

Because the rapid COVID tests we are using are now from several FDA-approved brands, and not just one, we’ve had to update our parental consent form in Skyward. Please fill out this new form (or re-fill it out if you’ve already done so) for all your students. 

The new form can be accessed by logging into Skyward Family Access, selecting Online Forms from the menu, then selecting COVID-19 Antigen Test Parent Consent Form REVISED. Please note: this form is not available in the Skyward mobile app. Please use either a desktop or mobile browser to complete the form.

Please remember that this new consent form is required for your student(s) to receive any antigen COVID-19 testing at school, including participation in the Test to Stay program. Please fill out this form as soon as possible so it is ready if your student needs it.

Rapid testing at school and efficient contact tracing can help us minimize the spread of the virus in our schools.
