How to Submit a Student Incident Report

- Sign onto the Komachin Middle School Website (top-right corner where it says "SIGN IN").
- Log in with your student username & password (just the username, not your entire email address).
- Go to the "FOR STUDENTS" section of the website and select "STUDENT INCIDENT REPORTING".
- Fill out the form and make sure to submit at the end. This will send a report to designated school personnel and administrators to investigate the report.
The Online Reporting form is the first step in a thorough process of investigating reports HIB, theft, drugs, etc. We want all our students at KMS to feel safe here. These reports are vital to this goal of student concerns.
