How to Submit a Student Incident Report

    1. Sign onto the Komachin Middle School Website (top-right corner where it says "SIGN IN").
    2. Log in with your student username & password (just the username, not your entire email address).
    3. Go to the "FOR STUDENTS" section of the website and select "STUDENT INCIDENT REPORTING".
    4. Fill out the form and make sure to submit at the end. This will send a report to designated school personnel and administrators to investigate the report.

    The Online Reporting form is the first step in a thorough process of investigating reports HIB, theft, drugs, etc. We want all our students at KMS to feel safe here. These reports are vital to this goal of student concerns.
