In order for students to participate in school sports your student will need to have a completed sports packet filled out, including a physical form that will need to be completed by a physician.
 
There is a one time sports fee of $60 to play middle school sports. It IS NOT a per sport fee.
 
If you are unable to get into your primary doctor to get a sports physical before the sport that your child wants to play starts you may be able to go to the Pacific Walk in Clinic. The Pacific Walk in Clinic address is 3928 Pacific Ave SE in Lacey. Their phone number is 360-455-1350.
 
Sports packets can be picked up at any time throughout the school year in the main office. The entire packet needs to be turned in prior to the first sports practice.
 
PRACTICES WILL BE HELD AT CHINOOK STARTING AT 4PM AND ENDING AT 5:30PM. PARENTS WILL NEED TO PICK UP ATHLETES AT 5:30, THERE IS NO TRANSPORTATION TO TAKE ATHLETES HOME AFTER PRACTICES.
