Hello, my name is Bonnie Harris, RN and I am the School Nurse at Lydia Hawk Elementary.
 
Please contact me if you have any important information regarding your child's health or have questions regarding immunization requirements, medication policies, or any school health concerns.
 
Cheryl Reams is the Health Room Assistant.  She is in the health room Monday through Friday.  She is very helpful and can reach me for any medical emergiencies.  Since Mrs. Reams is in the health room all week, it is helpful if you cc her on any email.

Life Threatening Health Conditions
Many students have health conditions such as bee sting or peanut allergies, severe asthma, diabetes, seizures or heart conditions that may cause a life threatening emergency during the school day. These students must have a plan including medication and/or treatment orders in place prior to the first day that they will attend school. Please contact your school nurse for the necessary forms or information.

Are your kids ready for school?
To attend school in Washington, students must show that they are fully immunized, or file an exemption with the school district. Make sure that your children are up-to-date on their immunizations.

Reminder about Medications

All medications that need to be given at school, prescription and over the counter medications (which includes medications like Tylenol, Ibuprofen, cough drops), must be accompanied by an "Authorization for Administration of Medication at School" form.  This form must be signed by both the parent and the physician/dentist.  Medication cannot be given to your child without this completed form.  Authorization forms need to be updated at the start of each new school year.  Forms can be obtained by calling the Health Room.

Emergency Info Health Cards

Each year the school requests parent fill out a new Emergency Information Health Card. Please be sure to turn in your updated blue emergency at the beginning of each school year. Health room staff cannot stress how important it is to have an updated card on file. Any time there are changes to contact phone numbers, throughout the school year, please be sure to update this card in the office. Thank you for your attention to this very important matter.
