My name is Diane Phelps and I am your school nurse. I am at Horizon's on Monday's. I also work at Olympic View, Aspire and Timberline High School.

Please remember that it is important to notify the health room if your student has any medical changes that could effect their care at school. This may include any new diagnosis such as asthma or diabetes.

Please contact me if you have questions or concerns regarding immunizations, vision and hearing results or medication concerns.

Angie Stockdale is the health room assistant. She is in the health room Monday through Friday. She is very helpful and can reach me if she has any medical emergencies.

Life Threatening Health Conditions

Many students have health conditions such as bee sting or peanut allergies, severe asthma, diabetes, seizures or heart conditions that may cause a life threatening emergency during the school day. These students must have a plan, including medication and/or treatment orders, in place prior to the first day that they will attend school. Please contact your school nurse for the necessary forms.
 
 
Is your student ready for school?

To go to school in Washington, students must show that they've had immunizations, or file a legal exemption with the school. Make sure that your children are up-to-date on their immunizations.

Please see the Washington Department of Health for a full description of the minimum required vaccines for school.
