  • Make sure your Medical Authorization Forms are in before school starts!

    Parents! If your child needs to take medication during the school hours, please have your licensed health care provider complete the medication authorization form. Bring both the form and the medication prior to the start of school this fall. Life threatening medications such as epi pens or diabetic supplies MUST be turned in BEFORE your child’s first day of school in order to attend. Doctors may fax over orders.
